TRANSITIONAL CARE

Our dedicated team of physicians, and physician-extenders perform post-discharge follow up calls. This service has been provided for patients who have been discharged home to ensure safe transition and enhance patients satisfaction. Post-discharge follow up has prevented unnecessary re-admission  and increases HCAHPS scores.
