POST-ACUTE CARE

Our Dedicated team of doctors and mid-levels will follow patients after discharge from acute care to skilled nursing facilities to ensure continuity of care, control of hospitals readmissions and resources management. We will follow those patients only if their primary care physicians don’t provide these services. We maintain excellent post-acute readmission rates and bed days.
